Responsibilities

The heart transplant social worker delivers specialized psychosocial care to patients across the continuum of advanced heart failure, mechanical circulatory support, and heart transplantation. This role requires strong clinical judgment, efficiency, and the ability to perform effectively under pressure while maintaining professionalism, sound decision-making, and reliable follow-through.

As a core member of the multidisciplinary team, this position supports patients and caregivers from initial evaluation through candidacy review, waitlist management, transplant hospitalization, and lifelong post-transplant follow-up.
